Is using a cheat engine to change body proportions punishable?

No I don't believe so. I have a few mods installed through Nexus mods to get different outfits for my character and it's been fine.

it is not punishable. i did it a while back on a run just to mess around. set to single player so i dont mess with anyone elses run of coarse

Only thing that you can end up punished for (yet still unlikely since its more or left done game) is game altering stuff like stupidly OP armour stats or weapons and generally anything that just ruins the game play it self.
