Missing Depth levels and Scout Vestige Part D

So I am still looking for part D of the Scout vestige, can anyone help me?

I currently have unlocked the following depths levels:

Den of the Dead
Den of Darkness
Town of Sacrifice
UPDATE - Swirling Flood
Swirling Flood
Cliffs of Rust

Silent White
Arachnid Grotto
Misty Ruins
Rocky Ruins
Void District


and I heard that there are two more, so I assume that one of those is where scout D is hiding.

Edit 1: Thanks to Dusty The Hamster I found now Swirling Flood - unfortunately that still leaves one depth map to be found, and since there was no Scout Vestige Part D on Swirling Flood, I assume it is there.

Edit 2: So apparently I miss the second-to-last map "Zero Ruins". If anyone could help me with this would be greatly appreciated.

I'm not that far into the game yet I don't think. Just made it to the boss in 'Ashen Cavern'. but I do have a Depth not listed there.

Flood of Impurity - if I remember this was in a 'zone transition area' you had to climb a ladder to get to a new zone but if you turned around and walked to the other side of the ladder hole it should be on the ground or in a chest like right behind the top.

Edit - I'm playing the PS4 version and waiting for a steam sale to rebuy here for the inevitable modding

Edit 2 - Cathedral first waypoint turn around and run back to the ladder but don't go down, just go around it.

some depths map is obtainable from side quest (you kinda need to sweep every area, after you progress the story a bit, and if you notice in the map there is a person with ! icon, they have some quest for you)

Pretty sure Scout D is on one of the maps you already have listed. I don't remember which one, but you must have missed it going through. I've got all the maps, and don't have any you don't.
But I have fully completed the scout and hermes vestiges so

Ran through them with the scout's item checker (which according to its description checks for items on the whole map you are on) with no results.

Scout D is in the one you're missing, which is just a pickup in the loading zone between Provisional Government Outskirts and Central. Once you go down the latter, look behind you to the left and it should be in the corner juuuuust outside your camera's view if you're running straight through.
