If there are any actual changes to the maps I couldn't find any differences in, I'd love to know. I'm curious to see what all they fix in these updates.
I just checked both Guillotine Worlds and both report that I get the Severed Head on interaction with the giver...

And here for Japanese vs. 12/1 update: https://www.reddit.com/r/yumenikki/comments/7pzqzz/summary_of_changes_between_010_and_010a_steam/
It is true that you end up with the Severed Head effect "item", but the text from collecting the effect in the large Guillotine World is still "Talking Head". See here.[i.imgur.com]
I should also note that the instructions on the Yume Nikki wiki for accessing the large Guillotine World are incorrect.
Stabbing the Toriningen does not have any effect on what determines which Guillotine World you are transferred to; it is a random 1/8 chance that you end up in the larger room, and is determined when you enter the bleeding zipper. (Notice variable 0026 here).[i.imgur.com]
