the game despawns the world behind you and spawns a new one in front of you.

story islands will simply respawn relative to you.
Yes, more stuff will spawn infinitely in direction your raft is moving to while things cease to exist in direction your raft. Essentially a world is a 4x4 square kilometer of existence centered on the 4 raft foundations you start the game with (they are essentially the center of the game world) and the bubble of existence follows your raft, not your character, everywhere (if you happen to jump on island and let your raft drift away, the island under you will eventually despawn, assuming your raft doesn't get stuck on something).

The game world is constantly being restarted, creating a sense of infinity. The islands are also being restarted. They change their position on the map
