Seems game bug. Not happen for me. But anothe one - research tree reset - happen if it shouldnt. I dot know how but saves/pofile file randomly corrupted.
Usually tht happen if some game files corrupted, you play another version not wipe old saves... well, or warez copy (and old saves).
If you done with campaign or only start it - same for survival and weeks progress - better remove game folder from My Games folder entirely.
Dont forget to turn off Steam Cloud for TAB (or entire in general Stem settings).
That mean start new game. For all modes.
Check game files. Better twice.
Each time I had a death ball of upgraded Soldiers running around clearing everywhere, each time I had them set to Chase when the bigger concentrations of enemies were taken care of so I didn't have to micro them. Each time they stopped chasing and then a few days after that, I'd get a spot at the edge of the centre of my locked down area get attacked by either a single Executive or 2-5 Fresh. It only has happened on those missions though.
Did you happen to check your power grid cells? That was the first thing I did, and on the campaign mission, I definitely had ONE cell tucked up against a cliff that was not getting power. It was visible, had buildings right next to it so I had line of sight on it. I had 4 (probably smaller than your 1) little death balls at each frontier, but because the map is so big and the zombie spawn happened almost like dead center of map... I had NO chance.
The second time I happened to me I made sure to pause and take a good close look before I got overran and lost. I am 100% positive that every spot within my walls were covered by Tesla Towers... and still these f*ckers spawn right inside of my base, inside of my walls.
Again, this seems unfair in an already very unforgiving game. I learned early on how easily a single infected can slip through unguarded frontiers and end your game, so I am very diligent with patrolling and walling off frontiers SOLIDLY. I often test my walls by blocking off the gate and trying to send a unit through- if the unit paths away from the wall, i know the wall is airtight.
Maybe now I just need to keep a ranger patrolling in every quadrant of my base? But that takes them off of the front... ugh.
Most likely you missed 1 zombie while clearing. If there is a single tile going "around the corner" or reaching into a forest line then sometimes you need to bring your units really close to reliable check if there is a zombie in it.
Maybe you could post a screenshot of the spot where it happened. Could help in getting precise answers :)
This was also a lesson I learned a while back, when I'm clearing areas I always send RANGERS (not soldiers) to run along tree-lines or cliffs; the Ranger's good vision range helps spot those infected b@stards tucked away in little nooks in trees/rocks. In my incidents, this wasn't the problem- I had been able to build many buildings along the edge where they appeared. If they had been there, I would not have been able to place lumber mills.
And as for a screenshot.... well, that wasn't really on my mind when this happened, as you can imagine me frantically trying to gather my troops and maybe get a chance at containing the threat!
For sure I'll be paying attention for this from now on. If it happens to me again, I'll pause and immediately get a screen.
I appreciate the suggestions! In a game as brutal and punishing as this, we need to pool our knowledge together to overcome the infection!
But this shouldn't be the problem here since OP has the little mouse icon next to their name, meaning they have a legit copy from steam.
as that has been a on-going trend as far back as 2017 of the occasional person owning a legit copy of the game yet the game "for some reason" thinking its a pirated version so that is a possibility

That's pretty amusing, but I bought this game back before campaign had even begun to be worked on!
Anti-piracy measures like that are way better than stupid nonsense like DRM. Ruin the game for the jerks stealing it from you.
