(I have this same bug but I haven't confirmed the timer)
I dug into it and here are my two pence : there are two bugs at stake.
First one is when the quest is active, you have a failure timer on it which indicates the fact that Sir Mutilator is fed up with waiting. Problem is that performing the duel DOES NOT deactivate the timer, resulting with a wounded Valerie telling you that he left.
Second one is that I suspect once the Valerie's quest is finished you are supposed to get a bar scene triggered next time you enter the town square. The loading screen is showing you a bar just before crashing instead of the settlement one.
As a conclusion :
1 Do the quest and finish it, the game crashes when entering town center.
2 Do the duel but don't mention the wound to Valerie, the quest fails and the gash remains. No bar scene ?
3 No quest, no gash, no bar scene ?
Speaking with her in the throne room dooms you to be stuck in it.
I spoke with her directly in the town center then headed to the bar to trigger the scene, nothing happens there but the game crashes when coming back to the city, with a bar loading screen as said before.
So I guess :
4 You can finish the quest in town square instead of the throne room if she's your regent, leave it and never come back to any part of the town if you really like Valerie.
I don't believe I've tried to go into the tavern directly. Usually I go to either the throne or the square. I wonder if that will fix the mix up? Thoughts?
[Edit] I don't like her that much.
I know the cost! Last night, gazing into that hearth, I saw things in the flames as well. I saw a king, a crown of fire on his brows, burning… burning, Davos. His own crown consumed his flesh & turned him into ash. Do you think I need Melisandre to tell me what that means? Or you?
Wish to see that bug fixed though.
Seriously, playing it relentlessly and then getting whiplashed by a bug is heartbreaking after such a wait.

If we could get a Valerie instead of 'Valarie' in the title of the post, that'd be great.
You can use Valerie just fine if you don't finish the quest.

Though I did not realize this had anything to do with Valarie.
Just finished the troll-quest, I go back to my captial throne room, then Linzi wants an audiance. She says she wants to speak with me at the capital tavern. I leave the throne room, then this error message appears. I did the Valarie quest just before doing the troll-quest.
Like you mentioned above, I think it cuts straight to tavern, jumping over the main square. It starts loading the square but then changes background to the tavern and loads again.
My playthrough is now completely halted, can not progress. I have no saves prior to entering the throne room. I cannot leave the throne room without generating this error message. Until this error is fixed, my options is to start over… which I wont.
