Companion released in Kobold portion of Old Sycamore Caves stay put at the gates.

Jaethal is the companion that I left last to recruit. After going through all the dialouge with the Kobold Chief and freeing the companion, the camera cuts to the cage and I speak to Jaethal. When the dialogue ends, I don't have the party select pop up to add her to my party. I already had a full party by now, if that might have impacted that. Afterwards, she stands in the gateway of the Kobold camp and repeats one bit of dialogue. She won't budge. Let me know if there's anything else I need to add to help.

I believe the last person you rescue in the caves won't actually join as an active party member until a little bit later in the story.

I remember in an earlier playthrough I stopped, I went to Sycamore first before Temple of Elk and ended up recruiting Tristian last and he was the companion that I couldn't use. But even still, he showed up at Oleg's and was present. Jaethal is stuck at the gateway and never apears anywhere at Oleg's when I get there.

Definitely could be a bug in that particular save then, she should have shown up at oleg's at the very least. She has for me when I rescued her at the cave as the last companion.

I guess the larger question then is if it gets fixed once you trigger act 2 or not (since you can't use the last person anyway, it wouldn't actually affect the game if act 2 fixes the problem)
