There is a lot of confusion with people saying "if you don't do X, they'll die" followed by another person going "I didn't do X and they survived". So either the whole companion fate system is totally bugged, or the guides fundamentally misunderstand how it works.
Upon checking the gave save editor, I can see the following flag states:
Nok Nok Jester: 3
Nok Nok Hero: 7
There is also Nok Nok Hero Final: 0 - idk what this do, but maybe this didn't change properly(?), although I did finish the "Hero, At Last!" quest, where he gets the statue.
The interesting part was however that when Nyrissa killed him she said something like "if you are such a hero then you can die one" or something like that. Well at least he has a statue, so ppl of the future will look up to him and think he was the hero of the nation and died like one xd
That is the flag that gets set when you finish the quest. So it did work perfectly well. Why Nok-Nok still died is not explained by the guides. It is possible that there is an additional check to see whether you have enough points. I think you can get a maximum of 10 hero points.
