Is Improved Initiative worth it?
Since I seem to eat a lot of damage due to opponents going first a lot, this seems worth it. Am I wrong?

For RT - no. Just run back for a round. For TB - yes.

I use turn-based. Okay, then on what characters? Buffers, debuffers, and ranged DPS?

I take it with heavy armor tank if they have too many feats. The first round is when they get hit the most, so i think it's a good feat for them since they tend to have a lower dex in general.

Flat-footed:

A character who has not yet acted during a combat is flat-footed, unable to react normally to the situation. A flat-footed character loses his Dexterity bonus to AC and Combat Maneuver Defense (CMD) (if any) and cannot make attacks of opportunity, unless he has the Combat Reflexes feat or Uncanny Dodge class ability.

Characters with Uncanny Dodge retain their Dexterity bonus to their AC and can make attacks of opportunity before they have acted in the first round of combat.

For TB... at least frontline would want it. And for sure all monks and that stuff would want it. Although it likely wouldn't help much for unfair and TB has better surprise rounds, so it can be circumvented. My take would be - ignore it in RT, unless you have nothing better to take and take it for TB, unless you need something crucial instead.
