I was a worshipper of Urgathoa for my first character actually.
Because the problem isn't lamashtu, it's the rest of the world.
The servants of all those evil deities are tolerated as long as they behave. Servants of lamashtu are usually executed on the spot in civilized parts of golarion. They don't like her.
If you were a priest of lamashtu, you wouldn't be in the room in brevoy waiting to start your quest, you would be in prison waiting to be hanged. Hard to play the game when the game doesn't want to play with you.
Lamashtu requires a special type of campaign revolving around it.
