Lamashtu?
Why is Lamashtu so fleshed out as a deity ingame but not included as a playable deity choice? Was it because it would be difficult to check for a player with Lamashtu as a deity when handling dialogue choices?

Yes, exactly. And also because her cult is illegal in most of Golarion. There's a huge storyline about these Lamashtu cultists, with the player uncovering their mysteries — would be weird if the player was one of them.
(Same for Groetus: most of Harrim's storyline hinges on the player not being a worshipper of Groetus).

But I can worship Gyronna(a goddess of hags and people cast out by society) and Urgathoa, who straight up hates most non-undead, are both pickable. That's what confused me about Lamashtu being a choice here. I think Groetus might be selectable too but he's not straight up evil.

I was a worshipper of Urgathoa for my first character actually.

Because the problem isn't lamashtu, it's the rest of the world.

The servants of all those evil deities are tolerated as long as they behave. Servants of lamashtu are usually executed on the spot in civilized parts of golarion. They don't like her.

If you were a priest of lamashtu, you wouldn't be in the room in brevoy waiting to start your quest, you would be in prison waiting to be hanged. Hard to play the game when the game doesn't want to play with you.

Lamashtu requires a special type of campaign revolving around it.

True, assuming you openly worship Lamashtu. Once you get your own lands, you can openly have a high priestess of Lamashtu as councilor or high priestess, so by that point, your character could come out as a worshipper of Lamashtu. Having numerous magical beasts and monsters hunting fey, creatures from the First World, and keeping enemies away from your commoners would also go a long way towards making Lamashtu more accepted among rural and civilized people in your barony.

the simple answer is they got lazy or simply forgot to add her as an option or didn't think it would be worth it. why and which one is the real answer we may never know.

Isn't KnockKnock follower of Lamasthu? You can appoint him as high priest...
