TBH her good ending is her rejecting the ways of her god, refusing to use necromancy, evil rituals, and to sparing her daughter. And then she will die after rejecting her god, during the daughter quest. But she WILL return in the end alive and inquisitor of Pharasma.
Not sure what happens if you give in to her old ways at some point.
Now I'm not sure what will happen after you enter the portal, because it also involves Tristian. In my game she decided to sacrifice her daughter and didn't die after entering the portal, but Tristian did.
Imo, everything after entering the portal is very poorly thought out, including the fate of the companions.
The ritual happens off-screen. It will either resurrect her and a new patron will take her in, or she becomes an undead (a perfect undead as she says). She is gone for now and will only return in the House at the Edge of Time.
Finishing the companion quests (often in a certain way) raises their affinity to you. Doing all in the "good" way ensures that no companion - except one - will die in the First World.
That's incorrect, Jaethal only dies if you favor Tristain, complete his quest without redeeming her and keep him around.
