She's your third option for the High Priest position, mainly to give Evil kingdoms a bone over Jhod or Harrim.
As well, if you kill her, the followers will give you some minor curse. Can't remember what happened after that though.
According to the PFKM wiki, she's also an option for Councilor [link below]. But I was more wonderng if she was a plotline or something.

Is that part of the Tristian plotline? I haven't been through it yet, but I've read hints in other threads.

OTOH, letting her do her thing(I.E. not killing/imprisoning/exiling her) counts as a negative during the street riots in act 2. It also costs you the minor bonus you'd otherwise get from the lamashtu cultists and of course your evil option for high priest.
To be clear; the benefits and costs of killing/keeping her are extremely minor unless you're evil-aligned and don't want Harrim to be high priest because he's neutral. You literally miss out on a small bonus/penalty during the act 2 riots(If you're "Good" you have so many positives you can take the penalty of keeping her around if you want to because you'd probably get so many bonus points from side quests resolved via "good" options it makes the riots a breeze anyway)
I play a lawful evil character, for whom Tristan wasn't an option, especially not after he betrayed me. Tsanna, likewise, was working to destroy my barony. So I executed them both, which seemed like the reasonable course of action
Why is it that all the characters available for the councilor position either leave by event (Shandra) or are enemies of the state? Did no one at Owlcat think that this might leave a lot of people without one?
It's not much easier as Lawful Good.
For example, I tried a paladin build that able to poison the Stag Lords wine, but couldn't mediate between the mites and kobolds at Sycamore Tree... and that was just Chap. One.
OTOH, I haven't had much of a problem with my NG Aldori /Rogue build, so I think there might be a neutrality bias built in. But maybe I'm wrong. /shrug

If you're arcana spellcaster (with access to 9th level spells) in Pathfinder - you already have spells that stop aging (from destroying your body&look, not for stopping it completely). So don't need gifts from crazy evil goddesess :)
Which leaves you with no councilor if you kill Tsaana.
