You didn't go into any of the caves on the Old Sycamore map itself?

go north, there the mites cave and at the end of it an entrance to another cave network.
Enjoy your stay, its the most frustating quest out there.
I couldnt find any caves except one mineshaft I couldnt enter and the cave network you can't enter.

I'm wondering this too. I've spent nearly three hours scouring that map. I've found "The entrance to a dungeon" but all it does when I interact with it is give me a description. I've read a walkthrough that said there's supposed to be a Kobold vs Mite questline going on there, but I haven't seen any trace of it. Like the big group that was supposed to start it didn't spawn.

Hopefully something is done about it. I don't want to have to start a new game AGAIN.

go to the pine patch which is an area on the world map north of the sycamore tree. You will find Tartuccio preaching to some kobolds and that will unlock the cave quest. Man, they suck at making sandbox games that put you on invisible railroad tracks!!!

Holy cow thank you, I was lost for a good while!
More bad game design. After you go to tomb, if you pass all the ability checks on exit dialogue it paths you properly into the pine patch and the mite / kobold event goes as normal. If you fail the checks the pine patch does not spawn (at least for me) until you go to old sycamore and go through the dialogue with the exile guy. Then you have to leave area and go back to pine patch to trigger the event line. I was stuck on this to on my third restart but figured it out due to meta knowledge once I saw pine patch on the map revealed. Maybe there is a hint to go to pinepatch somewhere in the dialogue with the exile but TBH after save scumming / restarting multiple times I couldn't be bothered reading through it again.

Happy birthday to this 1 year old thread. It looks like he figured out that the caves below the old sycamore tree are on the old sycamore tree map location, so I don't know if this is relevant anymore.

@Chief; This is pretty standard P&P RPG type stuff. You will occasionally fail checks, and the encounter in the Pine Patch isn't essential. The Pine Patch "spawned" for you later because you passed the Perception check to find it on your way by, it didn't have anything to do with going to the Old Sycamore.

If you don't enjoy the game because there is a chance of failure, then you can set it to Story difficulty, or cheat to max out all skills. Whatever helps you enjoy the game, but this is not bad design, this is what good freedom of choice looks like.

No it didn't, I specifically walked by all the routes around it trying to get it. It did not appear until after I went to old sycamore and talked to Garess. It showed up immediately after I talked to him and left the area.

Here are screenshots for proof - first shot you see I have been along every path, no pine patch: https://imgur.com/MtKkd5Z

Second shot is after scripted interaction with garess. I haven't moved from old sycamore at all, yet pine patch has spawned: https://imgur.com/vw8QEnz
