The river that you are supposed to be following is the "Gudrin River", and it should be in your quest log:

If it isn't in your quest log, then you almost certainly won't be able to find the Tomb (you've been bugged :( )
What is this ♥♥♥♥???
I have half of the quests not that you have here.
I have only 2 Quests...
The first (at the bottom) is "Attend the council"
Than the one above that is "Am Fluss nach Kesten und seiner Miliz suchen"
That is german and means "Search at the river for Kesten and his militia"
All the other quests i dont have... i dont have to defend the capital and other ♥♥♥♥...
How bugged is this game?
This rly pisses me off now...
I've completed the entire Bloom quesline, so of course I have more quests than you do. The key thing is the "Go up the Gudrin river..." quest -- you should have it, and it describes exactly what you need to do to find the Womb. If you don't have it (and Kesten has left the Goblin Fort) then you have a bug. If he's still present at the Goblin Fort, make sure that you've completed the Gobli Fort quests (the previous three quests in the screenshot).
Ok i tell now EVERYTHING
I was going to the goblin village where u get nok nok.
After this there was no other quest or whatever. I also made the quest where I search for the cultists. With the little detective stuff. And i made the operation.
After all this the council members gathered and they made the conclusion, that the seeds come from the river. Kesten told me, that he wants to go with the militia so i allowed it to them.
Now the quest says, that i should go search for him.
I dont have a quest for the Goblin Fort. Do you mean with Fort the Village where you get nok nok?
Well there is no flag. It looks exactly like your screenshot.
No red flag and i cant even click on it to see a discription. But hey at least i know now where to go. Thanks.
Btw. does this bloom stuff happen in the original Modules from Pathfinder? Does someone know this?

But there are so many bugs and what not we are testing and getting tired of.
Its kind of lame when players dont even BOTHER to write the text in the quest.
It dont only state what river, it state direction FROM your capital.
You wrote here before you read the quest text. Dont do that please.
If you aren't playing in English, this quest can be a real problem. The map looks to be in English, but the quest description (which includes the name of the river) isn't. And at least for the German translation, they actually tried to translate the name of the river (which is absurd -- proper names shouldn't be translated).
So, non-English speakers may have a serious problem here.
I am at the questpoint where I have to find Kesten at the river, but I can't get to that point you show on that screen. There is just no option for me the go southwards at the end of the lake like you did.
The Tome Of Lamashu pictured on the screenshot is where you go after the Goblin Fort. This also requires a perception check to add to the map (which you can't even attempt until the quest had advanced to the right place) and is easy to miss, because it isn't obvious that Gurdin river extends NE past Lake Silverstep. When you are at the right portion of the quest, you should reveal the location when you travel NE from Lake Silverstep VIllage.
