How to stop hotbar from changing on it's own?

Is there some way to lock my hotbar? It is constantly changing seemingly of it's own accord. I turned off Action-bar slots auto fill but that did not help. Every time I load into a new area skills disappear of my bar or swap around. It's incredibly annoying.

I have 'auto fill' turned off and run into the same.

I have no idea how this was tested before release - or - a way to lock your hotbar.
