Long story short, inv can get corrupted by failed attempts to save (my definite confirmed experience) or possibly by other unknown factors. Reattempting save until it succeeds will either fix the inv and all corruption (my 100% experience so far) or possibly half fix it while leaving some permanently bugged items in your inv (other people's experience). It's possible the latter happens after you perform interactions/quests/alterations with an already bugged inv.
Therefore, check your inv fairly regularly and ALWAYS after a save attempt. If your inv is bugged immediately after a save, then the save failed. Reattempt saving (until it succeeds) immediately after corruption and your inv will almost certainly fix itself. If you wait any longer or fail to notice the corruption for a while, then you might not get a full fix upon saving. Reloading an earlier save in that situation might be your better option, depending.
Using multiple save slots is recommended.
Restarting and reloading had done nothing to fix it. The worst part is, if I try to store the gears into the item box, they stay in my inventory and are DUPED inside the box. So I could technically fill my inventory box with infinite rainbow gears I can't do jack with.
I've noted something interesting though. The medicine seems to be completely impervious to bugs. The items which tends to be especially victims of it are all food items, especially mushrooms for some reason, consumables, as well as some equipement pieces occasionally.
So I would say the best way to not be affected is to rely mostly on medicine when possible, and try to not keep food around longer than necessary. Clear your food and bugs won't be able to get to it.
It hasnt for me so far.
My advice for this glitch remains: check inventory often, if you noticed it just after a save, then the game didn't really save - save again until it clears. That has worked for me: it was corrupting often after saves, but now I have 0 corrupt items and no inv corruption in the last several hours of play. If you can't pinpoint when it happened, go back to an earlier save if you can, or else saving might leave you stuck with a degree of corruption.
I've yet to have a save fail to save, using both item box and ''save anywhere'' (the later more often).
It's possible the 2 are related. Failure to save could easily lead to corrupted saves.
The save failures definitely happen as a regular bug of my game though. Well, I should say that they did happen. Since I started checking inv and re-saving, I've managed to go a long time with no corruption and no failed saves. Hopefully it's in the past.
When I still had the issue, I experienced the inv corruption before I noticed the save failure. I then noticed the save failure when the save I reverted to was a lot earlier than it should have been. Then I checked it out closely, checking inv right before saving and right after - inv was fine before, corrupted after, and then checking the save slot showed that the save didn't actually happen. This is when I discovered that successfully saving can fix the broken inv (at least for me, in those specific circumstances).
However as mentioned elsewhere, some people strongly assert that their corruption happened far away from any save points. So yeah, there could be multiple triggers (or one unconfirmed single one).
first is kiryu story around chapter 9-10 after doing daigo subquest and fighting some random mooks, my weapons and items turns into foods and stuff. because i fear that i might have a messed up save, i restart and reloaded pre- daigo subquest and all works fine again.
second time is with majima during chapter 11 after hostess training. unfortunately i saved and overwrite my main save before realizing that my items are bugged. but fortunately after restarting the game and trying to load it back, the "corrupted" save doesnt register so after loading the game im back pre hostess training..
mental note to keep making duplicate saves
Ran into the dupe bug after buying bugged pocket circuit items from Don Quijote (even though I hadn't even touched the circuit tutorial yet). Thankfully have a save not that far back, so not that big a loss.
EDIT: A previous save had a bugged Regular Gears Plus before I went shopping again so I'm not sure what to think now. It's only within my inventory so I can leave it alone for now I hope.
