Bonus portals not spawning when invoked

Playing multiplayer with my friends several times we've used the gold altar and it says "a gold orb appears" but no portal spawns. This has happened probably 2/3 or maybe 3/4 times we've tried. Additionally, we tried using a newt shrine earlier and the portal to the bazaar beyond time did not spawn, despite the message about the orb appearing happening. The orb still orbited the teleporter but there was no portal. This was the first time we encountered this with a newt portal. In multiplayer the newt portal has a success rate of 1/2 for us. We haven't gotten to the portal to unlock the mercenary in multiplayer yet, but it spawned just fine for me in single player once I reached the third world after the first loop.
