Item N'kuhana's Opinion Vanishes Forever

I was trying to acquire Item N'kuhana's Opinion, managed to kill the skeleton in the cave, and I thought I had got it already, so I quitted the game. After that I checked the item list and could not find Item N'kuhana's Opinion. And skeleton never shows up again eversince

1) Check the Log Book, Challenge section, Page 4, second in the list.. A challenge called 'Her Concepts' with the description "Find the Altar to N'kuhana' -- Is it marked as completed?

2) If '1)'=yes, Then go to 'Item & Equipment', 4th row down, 11th row counting left to right; Do you see a lock icon, a blacked out picture, or a red colored in picture? This is the slot that N'kuhana's opinion is at.

If you see a blacked out picture, then you have properly unlocked N'Kuhana's opinion, but have not yet actually found it in game, it's a red quality item. If you see a red colored picture, It's unlocked properly and you have found it previously..If it is a lock icon, but you have actually completed the challenge according to the first check in '1)', then you have encountered some form of bug for which I can provide no help. If it's a lock icon, but '1)' was incomplete, then this is expected behavior.

I should note, that unlike Runald's and Kjaro's bands, N'kuhana's opinion is NOT dropped by killing the creepy skeleton in the hole; this only unlocks the item to appear in the item pool in the rest of your runs, and you can only kill it the one time.

Edit: Oh, and, Once you've killed the skeleton that one time, it never appears again..which is intended.
