The point of the achievement is to demonstrate the trade off between staying healed up and maximizing damage output by outpacing REX's passive healing. It's a risk/reward system, letting you decide when to go all in for a quicker kill. You're not taking any risks by staying at 100% health, thus giving an achievement for that wouldn't make sense.
And yes, you are right, on eclipse REX is garbage (unless you're a masochist and enjoy pain and suffering)
