It's pretty clear it's designed to target your allies nearby based on how Dunestrider attacks allies and enemies with the Siphon that Urn is emulating.
Forget about the lore; no one wants this item except on entirely solo non-engineer runs.
I've given this issue its own thread in feedback, and there are bug reports on this behavior since it very much looks like a bug when it happens.

Sapping HP over a radius while low HP, possibly even giving you armor while active.
There are literally no upsides to the item in its current state that are worth the downsides, especially in Multiplayer.
A good Mired Urn would be something like this:
Falling below 25% health causes you to gain 100 armor for 5s and tether all characters within 13m dealing 50% damage and healing 50%(+25% per stack) of the damage dealt.
