has anyone found the log book for reformed altar

please tell me if you know where it is its one of the few items im missing for 100%

I got it from a Radio Scanner, not a book. Same for the second Path of the Colossus stage.

ok guess i just been unlucky since i havent see one yet
although i do have both variants of the second stage

only the last stage has a logbook, others have a radio scanner which you need to purchase, like many other stages
