why does a bomb keep insta killing me?

i only recently had this issue in singleplayer a bomb will attach itself to me after i kill a boss and it insta kills me and i have no clue why

Well there are a few reasons why;
If you have a sticky bomb and you have the Artifact of Chaos enabled, there is a chance you proc (activate) on yourself, and that would hurt, but probably not kill.

As Rat Bastard said, if it's a blue orb, it's an Overloading Elite, their attacks spawn little blue orbs that explode, but they shouldn't really kill you unless you're at low health.

You could've also been picking up the Fuel Array from the drop pod, which will explode after you reach under 50% health, what will kill almost anyone that isn't invulnerable.

yeah the fuel array strat is to use mul-t in multiplayer and to put the array in your second slot
