Could've been too slow about picking up items or something so you didn't snowball like nutters?
I know a lot of folks who prescribe to the 'rush the teleporter' philosophy without understanding the why and they get screwed over. You rush the teleporter because it forces the game to hand you money. If you don't spend that money, it's a bad plan. Once the game hands you tons of money anyways, there's no reason to rush or anything like that.
Don't waste time checking the entire level for items. Just get to the TP and run it as fast as you can. The longer you delay the possibility of being overrun the better. Once it gets really chaotic just keep your distance, try to stay airborne with feather so you're hard to hit. I had a run get destroyed by void reavers on stage ~16 before, keep a close eye out for them and keep your distance from them as much as you can.

What made me invincible was my brooches. I had a constant 4k yellow shield up from those.
It can only appear on Rallypoint Delta, and on Siren's Call iirc; you can use the bazaar to skip rallypoint, which just leaves sirens.
They'll nuke anything in range, player, boss, other void reavers, doesn't matter. So you just need to stay away from them, Or be very quick about getting out of the way.
To me, the biggest mistake I see in your OP isn't even that you got gibbed- IT's that you gathered so much movement speed that you lost control of your character. There's a point at which you should just start skipping extra movement items, Or converting them into scrap, because that loss of control can and will get you killed, either by a void reaver or by some other enemy.

They put in OSP for a reason... if their stupid achievements are too impossible for the average gamer, well that is not my fault, that is theirs...
