I've found one single pearl.

You can find "cleansing pools" laying around. When you give a lunar item to the pool you will get a pearl which boosts your hp by 10%, and a small chance to instead get an irradiant pearl which boosts all stats by 10%.

Also the rate at which they appear increases the higher stage you are in. (and if I am not mistaken are limited to 3 biomes)

I found 1 on Switch just by happenstance with the pool being in the same level as a lunar pod.

On PC the log entry remains unfilled.

I noticed if you play with more than 4 people the chances of anything but chests to spawn is greatly diminished. I must of played several dozen lobbies of 8+ and haven't seen a scrapper or anything above a roulette dispenser. it's all chests and maybe a legendary every now and then.

Two so far. One I intentionally farmed and one was completely by accident while removing Purity.

I found one before 1.0. Took a risk and stacked Corpse flowers along with transendence and ONE shaped glass...Maybe beads to i cant remember,bought my inital set in shop...I maybe cleansed 5-8 blue items and got one during that process...Boy i tell you that run was insane.

My first and only pearl was irradiant. Haven't found a cleansing pool since.

I've only ever gotten 2 pearls, both shortly after they were introduced, one of them was irradiant.
