How do 57 leaf clovers work?
As the title says, I've gotten some in my past few runs but have no idea what they do. The item description isn't quite clear to me.

It gives you an additional roll for proc items, using the best result. Ukuele has a 25% chance of proccing, but with a single 57 leaf that chance becomes becomes over 43%. Many good proc items don't have other ways to increase their odds, so that makes the clover uniquely powerful.

does it affect the shrine? (the one you give money and may give you random reward)?

Chests and shrines are under debate. It would make sense if it does, as shrines and chests are arguably the most important RNG element of the game, but people are leaning towards it not affecting them.
Partially because there hasn't been any super significant increase in drops with clovers, partially because prismatic trials have the same chest contents every time, meaning there's a high chance contents of chests and shrines are determined as the level is created, not when opened.

However it is hard to fully pin one answer or the other down due nature of RNG.

It has nothing at all to do with loot or shrines. It only affects chance activation items, with the exception of Tougher Times which it has no effect on.

When you have 3 clovers and still fail a chance shrine 7 times, it's pretty clear the clovers aren't working.

Believe me I have such amazingly garbage luck that it wouldn't even surprise me, but I agree regardless that it seems they don't affect shrines and chests. Still though, unless we get hard data you can't say with absolute certainty.
