That's not correct, for now. They have to make boss items and then make them drop different pools of items only specific to them, just like the 1st game had. The drops are still just pretty much random.
This item can only drop after defeating the Queen Beetle, replacing some of the green items that spawn from the teleporter.
The Gland spawns a Beetle Guard to fight on your side. This Beetle deals 300% damage and can spawn every 30 seconds, provided the previous one has died. This Beetle can block shots, and since it often runs in to the thick of battle, it might end up sponging damage.
The Engineer's Auto-Turrets do not inherit this item. It is not visually present on Turrets, either.

I just had it drop in the 1st stage and she was the only boss. ?
Also, it's a 15% chance of a green drop to instead be a boss drop from the teleporter, if said boss has a boss item (Horde of many and scavengers are the only ones without boss items)
But yeah queens gland can only drop when beetle queen is the teleporter boss
If you're using the artifact of command you can obviously pick your own items, queens gland included. But if you're not, you can ONLY have a chance at getting the Queens Gland if the teleporter boss is a beetle.
