its better to grab these early rather than later, i wouldn't pick one up after the 4th stage.
However it can also give you something insane like 21 soldier syringes, 9 of those crit=attack speed headresses, and 2 sac daggers. One of the most fun Merc runs I've had yet.
at least now i know wtf that thing does... i am so tilted.. fml
Here's what happened last time.
In a monsoon multiplayer run it saved my build. I got 20 Tougher Times and 8 Hot-poo Feathers. Paired with a glowing meteorite I dealt almost 70M damage and we stopped after 150 minutes because it was getting boring. But yes, sometimes it ♥♥♥♥♥ up your build. Use it in early game.
Sequencing ♥♥♥♥♥ up your run entirely unless found in the first 2 levels. The "lose gold on hit" item has the worst tradeoff ratio imaginable. Converting health to shields invalidates a lot of other items and it's very hard to not get hit for a longer time.
