Best is between glass and gesture...but probably gesture since its "downside" isn't actually that bad especially if you stack them and other cooldown reductions, but glass in the hands of someone great at avoiding damage is the single best power boost in the game. MUL-T can even afford to pick up one and half his health is the same as full health for the other survivors. But gesture has the least of all the downsides.
Hands down, the worst lunar item is Brittle Crown. With a chance to gain a fixed amount of gold (3), and a certainty of losing a scaling amount of gold (the amount of damage you took), it's basically the equivalent of opening up a black hole in your wallet. Second worst goes to Transcendence since it eliminates pretty much every recovery item in the game (brooch still gives barrier, and I think an Aegis would actually still get you barrier off of things like leeching seed, but stuff like fungus is still useless). Say what you want about Corpsebloom, Transcendence will get you killed more often due to the 7-second regen delay.
As far as equipment goes, my vote for best goes to Meteor. It affects the entire map, and it actually does do a very good amount of damage. Having to dodge the circles is a bit like having 10 stone titans on the map at once, which will happen eventually anyway.
Worst equipment goes to Effigy of Grief. Small area of effect, requires you to manually pick it up, and affects you while you do so. I have yet to test the Spinel Tonic, so that was not included in my consideration.
best has to be the spinal fluid combined with gesture. having those 2 plus soulbound catalyst = you have almost no need of any other item all the way into late game, you could have just hit a shrine of order but you get these 3 things, your run is instantly back on track.
Overloading worms by the 90 min mark can be so much fun. Had 32 hp including the topaz broach if I am not wrong, was playing as commando. Went for 4 hours before ending it.
I picked up 2 in a run when i had 11 glass since any hit would've killed me anyway, and it just went towards increasing the amount of money I had.
Some of the bad lunar items are the corpsebloom, meteor and effigy.
Probably the worst is meteor, as it is pain to dodge, especially when you have 7 fuel cells and 3 gestures and it activates every second.
Worst by far is Corpsebloom. On the bright side it becomes moot if you get Transcendence. Any other time though it's a runkiller.
Something like Meteor can't be the worst because you can just not use it. 0 effect is better than negative effect. Compared to an item that's always bad and that you can't turn off, it's just nothing.
The best Lunar Equipment is hands down the Hellfire Tincture. Early game (with no items) it's the strongest equipment in the game. You can easily do 3-4x the damage of a Preon blast with one use of Hellfire, and the cooldown is relatively short as well. No item scaling though, so you drop it once your build gets going. It also conflicts with both of the best lunar items (trans and gotd).
Gesture=Second Best-Good with Capacitor and DML
Corpsebloom=Worst-Just believe me
Brittle Crown=Second Worst-C'mon printer, CHUG! CHUG! CHUG! CHUG!
Blue Meteorite=Best Trolling item+portable dodgeball
Effigy of Grief=This+Huntress is enemy cancer XD
Transcendence=Best Defense Item
Hellfire Tincture=Best Crowd Control, Hands Down
But the worst lunar item is Transcendence by far >.>
