go to "Settings > system > display > Graphics > Default graphics settings" and disable Hardware Accelerated GPU scheduling

It's blurry on some of them because of the render scale, but you can see there that, despite being on the lowest of the lowest settings, something seems to be off in GPU frames/dropped frames.
Edit: It's not on the lowest of the low there on the screenshots, I reduced shadows to 5m, which didn't change anything but I think you get my point. Just checked my drivers install, and also reset my video settings in nVidia.

can you maybe give ALVR or Virtual Desktop a try?
VD is paid, $20, but ALVR is free and open source.
Are you on a 6g router? if so, where is it in terms of location to your headset?
I'm not too hopeful for Virtual Desktop, but if it's still happening after a few more attempts to fix and there seems to be nothing else, I might bite the bullet and try it for two hours and if it doesn't work, get a refund and hope someone can resolve it someday. It could be the tiniest thing I'm overlooking after all this time trying bigger fixes.

Looking at this video, I can see someone is running an older version of the game (I assume there's even been performance enhancements since then) on higher settings than I can, with no dropped frames and a very good last frame time at 7ms. Something must be wrong with my config or settings at that stage surely, but I seem to only have issues in this game, at least significantly noticeable ones compared to other VR games.
I truly have no idea, my game will hit 90HZ fine, but anytime I seem to do anything, frames will jump all over the place. Opening my spell menus, just having another character fighting me, or my inventory it really seems to take a toll on my performance.
