However for your specific issue, it kind of sounds like your are having the beginning stages of headset issues... Is your VR headset working well with other VR games that have a range of motion like B&S? I used to use a Rift and the HDMI cable was notorious for breaking, and it started with similar behavior as you are having where it would cut to black until eventually one day it never turned back on. I went through 3 HDMI cables with that headset. So if you didn't already, I would double check other strenuous VR games (because you want to test a game where you are moving around and causing the cable to jiggle like B&S, etc) and play for 15 mins or so and just see if it is not happening across the board.
Another worst case scenario it could be was something I experienced with my Index but using Vive sensors, where the sensors just one day gave up and lost tracking. The motor in the sensors burned out and refused to track, so it was like the headset could not be found and I had to replace the trackers.
The reason I'm wondering mostly about your VR hardware is because if the game got screwed up the likely thing you would experience is infinite loading, or worst case a crash back to your desktop, but I can't imagine a scenario where the game causes your headset to cut out, so that's why I'm a bit worried it could be something worse than the game. And as well as that, it sounds like you had some good troubleshooting of the game from your description; you did the right things with exception of just that one extra thing you can try that I mentioned.
What you mention here was interesting though:
Do you mean that the game is not showing a mirror on your desktop monitor? Or do you mean your monitors themselves are not working?
RE: possible cable issues
After posting this I played around in superhot vr for an hour or so and didn't have any issues. I was up and down from the floor and spinning around so I feel somewhat confident that it wasn't due to my movement/the cable
FWIW I'm using index with the 2.0 base stations and the knuckles (the full index package from valve).
When the headset image goes away if I quickly take it off I see my monitors lose signal and I get the "no signal detected" message on them. Now that I've thought about it some more it sort of seems like a driver crash. My computer then doesn't react to any inputs and so I reboot using the power button and on start up the Radeon software says the typical "Default Radeon WattMan settings have been restored due to unexpected system failure".
It looks like there is an optional update for the GPU driver so I figure I may as well try that. I'll let you know how it goes.
I saw that there is a log file in MyGames/BladeandSorcery/Saves/something so I'll post that in a bit if this doesn't resolve it. Is there a particular log that may help?
Ah that's great! Yeah it had that vibe alright, when you were saying about the monitor thing. Glad you figured it out.
