PLZ help my npc spawn spell is horribly broken

when ever i try to spawn an NPC there's 50/50 chance spawn a absolutely terrifying monste r instead.
IDK if I can post picture here, their eyeballs, mouth are fully out, their limbs are stretched out and they won't do any respond. they're basically immortal.
And everytime I have this problem I try to reload the level and it'll stuck at 88 percent
