Tomb of Annihilation: "Death Curse" mode
It's no secret that the driving force behind the Tomb of Annihilation storyline is the "Death Curse": people cannot currently be resurrected from the dead, and those who have been raised from the dead are slowly wasting away. So, as a "hard mode" for the ToA campaign mode, what if the Champions were to be under the Death Curse themselves? That is, if any of them drops to 0 HP, that's it -- no coming back to life by changing areas. They only way they come back is by resetting, or possibly reaching some milestone (e.g. destroying the Soulmonger). There might be the option to swap them out of the formation for living heroes, and/or maybe move them around in the formation, but that would be about the only mid-game way of dealing with them.

I like this a ton. Quests can be difficult, persnickety, and demand more active play, because we only have to complete them once.
