You can get the Extreme mode achievement even if you swap it lower mid game.
You just have to kill the final boss on Extreme mode and remember to be in extreme mode at certain points to unlock Liz's blacksmith level
Doing EX below 75 IS DO ABLE, just be prepared to die unless you're a freaking master at the game and know all enemy movements and can just dance around them.
Since you've already restarted I would suggest just going around and killing stuff in the first map as much as possible and doing ALL the sub/hunting missions you can until about lvl 80 or 85.
around this level you should be ok to fight in the cave and take a few runs to do it but should be able to do it.
and yes a great way to level now if online co-op since exp multipliers have been increased like crazy. this will help a lot at lower levels but i feel like it will drop off at around 180-190 all the way to max level.

Don't worry of being underleveled unless it's the DLC missions. You can take the final boss of the main game at level 120 even with subpar equipment
