I for the life of me cannot find the Makai brothers in the Demon Cave (Makai Type XX and Makai Type XXX). I've ran through that dungeon multiple times. What am I doing wrong?

it's in the depth, the second teleporter beside one for original boss just like forgotten temple

The terminal that makes it a fast travel point is straight across from it and not the one down by all the other computers, that is the one for the normal bosses.

that second teleporter is red, how do I activate it

You have to defeat the bosses the first time in the DLC and in order to do that you have to get the 5 passcodes from around solitary sands once that is done the second teleporter will become available for the quest.

In other words you have to have finished the new DLC before you can open the second teleporter.

I think I read somewhere that it needs to be activated for the first time while on Extreme Difficulty. This would be consistant with some of the other extra bosses in the games, though I can't say for certain because I always play on Extreme.

I can confirm that it doesn’t have to be on extreme difficulty as I just wanted to speed run it the first time for the story so I ran it on normal the first time.

No I can confirm the requirement to unlock the second teleporter in the forgotten temple and the demon cave is to finish the second DLC.

You have to get the home interior upgrade from Harry. (He should be at the shop where you first meet bazalt Joe). Then you have to talk to premiere in one of your rooms to get the quest about the Makai Brothers. After that, the teleporter is available for you.
