Affinity level 4 100%

is there a special event that triggers if i get the affinity percentage at a 100 or do i just need to hit level 4 in general

There is something like where you lay in bed with 'em or something, but i have no idea how to trigger the event.

level 4 0% is enough for the achievements.
4 75% with the GGO characters is required for the true end and all scenes.
4 100% has no benefits.

Do you need to be level 4 with every characters, before you can get the events?

I'd have to do another playthrough to check, but I'm pretty sure the requirements for the true end are the same as all scenes. That is, 'old' characters at level 2, GGO characters (except free dlc) at 4 75%.
GGO characters are Kureha, Itsuki, Zeliska, ArFA and Bazalt Joe.
DLC characters are Llenn, M, Pitohui and Fukaziroh.

no event triggers but lvl 4 100% is needed if you wanted to change the selected npc's outfit
edit:
"new" sao charas are in pve lobby and "old" charas are in lobby normal mode.
sao anime girls are in kirito mode lobby

Only Kirito gets dates with the old characters. Keep playing until you get to Kirito mode.

with the sao anime characters, you have to be in kirito mode and find them in the lobby,
with the game characters, the "old" charas are in the lobby normal mode while kuehra and zelisha are in the pve hunting lobby room
