Most people would recommend against doing it honestly. Your affection would reset, you'll also lose your weapon arts, dual wield, and sword+gun. I think there's something else you lose.
The only upside is you unlock Lisbeth's rank 7 upgrades for extreme mode by progressing in the story. The dungeons I'm pretty certain you'll unlock just by switching to extreme in a rewind, so no need for NG+. It's pretty much just for Lisbeth because her upgrades unlock with story mission completion. So if you're adamant about it, then you should probably get the true ending first, so that the loss of affection, which is by far the most annoying grind, wouldn't matter.
Im using extreme mode but what are the dungeons that are unlocked? I cant seem to enter forgotten temple

Then why are you asking how to start NG+ if you have already beaten it. You should already know how by default since you know "I've actually beaten the game". So anyways you start NG+ by defeating the final boss and choosing the "NO" option at the end instead of "YES".
I imagine with him (the topicstarter) no longer replying here he probably figured it out a year ago.

But to answer your question.
IF you want the true ending you finish the NG+, After that you have to start the story mode over again.
If you allready have it just dont finish the story mode if you have all dungeons etc unlocked.
That way you can just play on.
