At 1:12 (in the video) you can see the saber flying completely out of the playspace.
I tried rotating/moving the playspace, swapping the controllers, cover reflecting surfaces and even disconnecting one of the two base stations to see if one of them is faulty... but nothing, still always the right saber loses tracking during fast streams. So it seems very unlikely to me that it is a hardware failure.
I'm using the HTC Vive Wands.
Edit: Also i noted the controller stops vibrating when hitting notes 1-2 seconds before it loses tracking.
However i found that holding the controller in a different way will help you to not lose tracking (that often).

For me, the yellow one (3rd) is the best for not losing tracking and still being able to hit very fast and precise.
Additionally, if you notice the controller stop vibrating, you can slow down your speed and hit the next few notes very gentle until the tracking stabilizes itself again.
My config has not changed, and when 1.3.0 dropped, I had perfect tracking and was demolishing my high scores. This is on a Valve Index using four 2.0 base stations and the Index controllers.
Lately, I actually am scoring better on the Oculus Quest, which is really sad. This is on songs I've played to death, and previously full combo'd.

Restarting the controllers or switching batteries will sometimes fix it temporarily for me.
How does this just trail off a year and a half ago?? What on earth I am losing my mind with this game now, I tried so many tracking things but it didn't make any sense, always the right controller even when I flipped 180. Worked fine yesterday and is hosed today. I am losing my marbles with this!
Anecdotal test I found myself doing: try a restart and running the game with as few USB devices plugged in as possible (I would only have a keyboard and the headset plugged in). If you see an improvement, your USB controllers might be hitting their limit and causing the problem. Aside from that, if you're using an oculus, support told me the controllers can't handle hard & fast swings (spring compresses and battery disconnects momentarily). Those were the 2 biggies I found after months of trying to resolve the issue. I eventually did get a USB expansion card to try giving the headset its own dedicated USB controller, but by then I had already given up on the product and wasn't able to verify it resolved the problem, lol.
