What OS, graphics hardware and drivers? It is possible that the graphics hardware or display is not capable. For example my HDTV can display at 120 fps, but that is not real, it is interpolated from 60 Hz input to smooth and sharpen video to reduce motion blur.
After you manually set your refresh rate at Windows then Portal 2 will run at the set resolution if it’s Windowed. If you want it to run at the set resolution Full Screen you have to re-open the game.
