About those three difficulty settings
So, if we have three difficulty settings. Can I swap through them on the fly? Say if I was on the Normal difficulty, and I perhaps went onto Hard next. Could I perhaps swap straight back and the enemies would scale right back down incase I literally bit off more than I could chew?

Plus, dependent on the difficulty does it add/decrease any rewards/drops in any sort of way?

I think so and I guess it will increase the chance to get more and better rewards.

Difficulty only changes experience.

I think I may have also heard that the difficulty level may only affect the bosses as far as quality of drop. This is in addition to the xp. If I remember correctly the harder the difficulty the larger the pool of loot it can draw from. Again though this may not be correct so take it with a grain of salt.

Changing difficulty will reset your world and campaign progress. Not a big deal since you'll likely see less than 50% of the content in a single play through anyways. The reason for this is things related to the difficulty are generated when the world is generated.

Is it feasible to start on Nightmare difficulty with a fresh character?

No, you will run out of ammo. Nightmare is meant for high level characters.

I'm working on a summary of GG's Reddit AMA. You *can* start on Nightmare with a fresh character, they let you, but clearly they don't recommend it. :)

Nightmare is meant for when you have beaten the game and have end-game traits... and you are powered up. It's not meant to start on, but we let you do it. Enemies are tougher and more dangerous (and more aggressive).
So it's a not a Diablo-type system where that difficulty is "locked."
