If you have a lot of not upgraded stuff the game will think that you still need the most basic iron, the same for each tier.
So we must upgrade everything or most of our stuff. With one character I am in the second run on Hard, level 250 aprox. and I am still not finding Hardened Iron to upgrade >+15.
The closer you come to +15 the more hardened iron you see... Thats how it works.
I still have tons of stuff that is lvl 1-5. Now a day later I have max level gear. It has to do with your gear level when the area you went into is generated. I'm starting to think its only the very first generation of the area. Leaving and using a stone to reset the areas wasn't having an affect for me.
I was rerolling my campaign frequently looking for one specific boss. Each time I ran though to that point and didn't find the boss I would go upgrade my gear again( trying to keep them as even as possible .. ie: all at 16 before putting something up to lvl 17) and then reroll my campaign. Each time I went into the areas for the first time for that reroll there were more and more hardened stone every reroll because I was leveling up my gear between each new generation of the area.

Yet I never upgraded any of my other items in my inventory. I only upgraded my main equipment ( primary and secondary guns, melee weapon, 3 slots of armor) and from rerolling I ended up getting more and more hardened iron to show up after the first initial reroll. If it is as that post says and what you say I should not have had as big of an increase in hardened iron showing up in my world as I did. It's to the point now where everywhere I go I see tons of hardened iron. I still haven't leveled any of the items in my inventory up and I have added more items to that list.
So either that post is wrong or someone doesn't know what exactly they are talking about.
The link they mention clearly states that it uses the highest level equipment you have for each slot to create its weighted average, *NOT* what you have currently equipped. This is *specifically* to prevent people switching equipment to generate lower level areas before switching to their leveled up equipment to breeze through.
