Seriously??? You sure your not confusing it with Hardened Iron cause i have like 600 forged iron lol. As far as OP Goes i think i started seeing it in Rhom I dont honestly remember but i didnt even need to upgrade other crap just what i was wearing i didnt need to start upgrading everything else till i needed Hardened Iron.
Basically you need an item in every slot to be +5 and then generate a new area - it should be available to drop or find laying around in that area though somewhat rarely.
Once you start getting pieces slightly higher itll come more frequently.
My character atm hasnt even stepped on rhom and is getting galvanised iron now (playing with a friend, weve been resetting earth for events and stuff)

Scaling: the highest level equipment + 1
Materials drop: average level of all equipment combine
If you have everything +5 then total average level is 5, which will increase drop rate for forged iron
Check the vendor as well in Ward 13, she started selling some after a while.
Yea pretty much your upgrades determine the drop range. I haven't noticed anything with trait level, went to high trait level with no upgrades and the drops didn't seem affected on one character.
If you are missing upgrade items, its best to just join other games and just run through a few maps as they will usually drop higher stuff (Or at least that has been my experience).
