Well, I know some of you guys feel really passionately about this port (which is understandable) and might think posting outside of disapproval might send the wrong message of your stance, so I thought maybe putting that into the OP might help clear up the thread or make them posting here easier or something. :)
I'm not sure I entirely understand due to the Lavos mention. So I first go to the Black Omen in Present Time and go to the Queen and beat her, then escape and head to the Medieval Times and jump back into The Black Omen, beat the Queen again, escape and then jump back to the Future (apololypse) and then do it once more and escape, correct? I should never actually make it to Lavos, right?
After you beat the queen in the present, you get to fight Lavos' exterior. After you beat it, it remains dead, regardless of when you come to try and fight it again, and you get the chance to escape. After you escape, go back to 600 AD and do the whole Black Omen again until you get to Lavos' husk just waiting open. Escape again, go back to 12000 BC and do the Omen one last time.
The idea is that, since you beat the Omen in the future (present actually) it remains intact in each of the past instances of it, so you have another chance to go through it and get everything again when going back to 600 AD and then again in 12000 BC. So long as you start in the future-most instance of the Omen and then go back one time period at a time.
