start or load a scenario. At the bottom of the pause menu there should be a "censor nudity" box. Make sure it is not checked. If that isn't the problem I don't know.

i just loaded the game and in the start menu under settings censor nudity isnt checked

For those who are having problems with the explicit content add-on DLC not working make sure you go to the game properties in the steam library and check the box for the DLC. If you can't find the DLC make sure steam preferences is set to show adult only content.
