Don't know if it's available from the start, it may be like Lost Song where you basically have to clear the first zone to make your personal character.
Literally the first thing you do in a new game is create your avatar.

You can make any changes by going to your room at the inn.
per above. 10 seconds in and youre making a character. everything can be changed including your name at any point thereafter, except for your starting weapon obviously (and literally the only thing this affects is which weapon you start with and which weapon skill you start with, by the end of the game it will not have mattered because everything opens up)

As said yes character creation is right at the start, however during story cinematics your kirito just so you know.

The character creation only changes how you look and sound when you actually play the game. During cutscenes, your voice will always remain as Kirito and during image & cinematic cutscenes, you will always be Kirito and not the character you created.

Luckily, this BS is done with when SAO: Fatal Bullet comes out since the character you will create in that game will be the main character.

That's good news. Otherwise, the way they handle it here feels off and immersion break-y.

You are unable to switch to any other character besides this character in game. To make up for this, you are able to create your own character from scratch, and alter the name and appearance at any time in game where you can access your room in the inn, in the market square. You can also use any weapon in game and build a custom character from the skill tree.

For additional party members, you can have up to 4 party members at one time(Including yourself), and 3 additional NPC party members. You can add npc's by completing their quests, improving their relationship with you via in game dialogue, and waiting for them to messgae you with an additional quest, normally one that would take a little longer because they require something rare/difficult, and once your relationship rank with them levels up to 3(You can check thir relationship stat with default "H" button, it will show the rank at the bottom), you can use the default "U" key to talk them and request them to join. You can have up to 3 differrent groups(Max 12 players) in raid missions with bosses. Keep in mind that npc's will die permanently when killed if I remember correctly, though none of the main story characters will die permanently. In addition, NPC's will either be in town OR out in the field fighting monsters, so keep that in mind. Also, NPC parties out in the field that are not a part of your part(As far as I know) will not die permanently. Keep in mind I'm still not 100% sure if NPC's can die permanently at all, so take that with a grain of salt.
