I'll echo Agent Orange and say that a remaster for the first game would be good to see especially if they could polish up some of the rough edges. Alternatively maybe a bit of DLC for this game would be nice to see, there's lots of characters that seem like they would be interesting to explore more but don't have a huge amount of screen time.
Personally I believe that the line about him having a kid was a lie/excuse to run away.
What I would absolutely love to see in the next one is some choice and consequence added to the game, I'd like to see mental connections expanded upon for this but also just dialog choice and consequence as well. Your various choices could have a negative or positive impact on your own brain - and maybe as another character (Lili perhaps) you have to go into Raz's brain to work through the consequences of the choices you made.
It would be cool if they moved the plot ahead 5 or 6 years, and had Raz dealing with some even more trippy issues. They touch a little on death - but I think 5 or 6 years in the future, some of the original Psychonauts might be on their death beds and I think those feelings would make for an interesting narrative arch within this universe.
They could even have a brain that is loosely based on the aesthetics of Grim Fandango...
They do be having that Microsoft money now...
