I got it by being nice to everyone and giving the crown to Duncas.

Maybe the same ending but you are missing one or more of the factions because of reputation?
Which would be my main guess.
Either that or you are not supposed to have all your companions at the end.

If you dont do his personal quests you have to fight and kill him during the coup.
Jup that is what I thought of. But I saw someone complain that Siora died at the end of his/her playthrough. I guess you can generally just avoid the quests of one or several followers and get interesting results.
