You cannot enter the temple more than once.
You missed a major hidden TLB, here is a spoiler on how to get it:
Get Temple to spawn on 4th floor, clear ALL paths in it, then activate the warden when all 4 torches are lit.
Beat him, then prepare for another big boss fight.
You'll need to play on hard mode if you want to fight his final form and get the second banish achievement upon beating him.
Other questions you had:
- You looped for the first time. You can change loops at the mods computer at the hub, by scrolling all the way down. However, I suspect that because you simply quit the game instead of dying naturally, the loop feature didn't unlock for you. (did you alt+f4? that could also be it)
- Secret cartridge grants you access to the floor after Monolith yes.
You shouldn't encounter it anymore once you start looping, since it will become available automatically.

Damnit, i was so close to actualy meeting the TLB then, i entered the tier 4 temple and almost beat up the warden, but died at the same time as him.
I didn't alt+f4, i just went to "quit to menu", because i wanted to see if i could find anything new in the hub area. ( and since i've read that you could choose loop, i thought it would be safe to leave )

Edit : Oh my god... Even if we DON'T enter the temple, it won't spawn in later floor... Welp, so i'm forced to take damage or bomb to reduce my multi, which suck quite a bit.
Just try again I suppose.
I didn't entered it, and i killed the 4th boss with a x3.0 multiplier, it didn't spawn.
So there's definitely another requirement in play. I did it again on my run, but on "mild". it didn't spawn again, but this time it never spawn to begin with. So maybe intense or harder is needed?
...Or maybe i took damage much more easily because i went pure YOLO on mild.
I'm curious as to what the other requirement is, unless it's the flawless floor one.
I understood that, i managed to enter the temple one more time, but sadly, died to the warden before i could meet the TLB this time.
Flawless is definitely easier on intense mode, since you get more damage and much more occasion to get a shield. So it may be a requirement?
Leave yourself some room to see what's happening, or it will be a short fight in which you didn't get to learn much.
Definitely not a requirement to get it to spawn anyway. However, since on intense you progress much faster, then speed could be another condition.
For me it always spawned when I flawless a floor while covering a good amount of rooms fast enough. I could never put my finger on the actual requirements until someone said on forums that you need 2.0x .
I'm fine keeping doing intense, tho. Been playing the whole game like that so far, even if it's.. Incredibly more hard to reach the TLB this time. Well, if the TLB truly is an absolute nightmare when i met him, then i'll try to fight him on mild.
So, what we know for sure :
- at least x2.0 multi
- Flawless floor ( it spawned much more when i played carefully. Even when i didn't enter it i could see it again. )
- 4th floor for TLB

-Speed
To be fair, i doubt we should beleive the trailer; They could just have changed the trigger to make it so it happen anyway without doing anything.
But, really, "x2.0" and "flawless floor" are pretty much the same, in a way. I didn't check if using bomb in fight does remove multiplier however, but i think they don't. So in the end, it's still "flawless" in the end.
As for why I suggested dropping Intense lethality, well.. I'm a fan of the setting myself too:

But I also know better after fighting that.. thing. Biggest waste of time trying to ace it blind. Unlike Monolith, this is more of a gauntlet when you pile up the Warden on top of it, so taking out both with 1hp + shields / bombs is unlikely.
But it's your call anyway, I've seen some broken weapons that turn the entire game free too, so there's that.
Oh, i'm not going "sudden death mode", but the one in between : intense ( 5 health, 150% damage )
I do try some sudden death run from time to time, but since the DLC is out and i'm going on unknown territory, i'm sticking to intense. I'm used to play bullet hell with only 5-6 lifes anyway, so intense fit me the best.
But still, a blessing for the temple, eh? That's really good to learn, but must be pretty rare. ( Or i'm just unlucky! )
Still, someone like you did have problem with it? Now i'm even more excited to see it.
Then yeah, playing on 5hp is totally fine, since you sound experienced enough, and I did ace it on 5hp as well.
Although... if you really want to to almost guarantee a blind clear, then you can use the support pack that you buy from the cat, which will generally start off your run with +20~30% damage boost, along other pickups, but that's totally up to you.
I've only seen the temple blessing once in the 10h I spent on the DLC, and I've come across a good number of shrines, so either rare or just dumb luck.

Heh, i'm not as crazy and strong as you for now.
As for the care pacakge, i'm waiting until i can manage to clear the 4th floor un-damage everytime. If i can't do that, then i'll just have waste some credits multiple time. Though, i guess i should still do it one time to re-unlock the loop faster, maybe. ( since it truly seem to be gone for some reason. )
Blessing are pretty rare in general. I alerdy had some runs with 0 blessings, and some really lucky run where i had 2.
Anyway, i'm sure i'll get to him eventualy. The hardest part is just the 4th floor flawless clear, really. After that, it's just about memorizing the pattern. Thanks tho.

The temple spawned.
So "flawless floor" is not a requirement. That, or "under a certain amount of hit", at the very least.
Edit : Ok,i tried it again on the 4th floor, i took one damage, but did flawless on the boss ( no bomb, no damage. However, i did got hit since i had a shield. )
Only onticable thing in both those floor : i did the full floor as usual, i had the multiplier above x2.0 when the boss died, i didn't took damage on boss, i didn't used any bomb on boss.
If i had to guess, i think the requirement would be :
-x2.0 multiplier or above
-no bomb used in the fight
-no damage took.
2nd edit : I just discovered something stupid, but important for speedrunner...
After the boss battle, you enter a small cutscene to get to the next floor...
If you press "space/skip" the MOMENT you hit the middle of the screen, the game will give you a small "Perfect!" and make the screen transition instantly to the next floor.
..Maybe there is something else to gain from it, but a nice detail.
Edit 3 : I've finaly met him.
With 1 hp left, 0 bomb, and my starter weapon. ( cause the temple didn't give me a single health drop, a single ammo drop, a single weapon, or a single ♥♥♥♥♥♥♥♥ shop. I was extremely unlucky on that one... )
He look extremely fun to fight.
