You can't. You can change governments for your original flag and name, but that's it. Release the country you united with as a vassal and liberate them.

TECHNICALLY, you can dismantle unions, but ONLY by forming countries. For example, if I was South Korea (to which you can form the Korean Empire) and I made a union with China, then became Korea-China, I could then just form the nation Korean Empire and become just Korea again.

I don't know why South Korea can't form Korean Empire in this game even the game made it form-able.
Please give a try and let me know if you can, but I believe you can't make it happen.

Necro wasn't necessary. To answer your question though, to form a country you need to have enough gold, be independent and not be in a war. There are some other factors as well I believe.

not to be a necro or anything, but as of currently, playing south korea doesn't actually give you the option to form the korean empire, atleast from what i can tell.
