I suggest the Seduction mod, so you at least have an option if something similar happens.
It gives you the option to seduce someone to get an illegitimate child, that you can then legitimise. You end up with a bunch of negative consequences, but at least the game wont end with the lack of an heir.
Wanton would normally mean "having lots of sexual relationships" so that was probably a malus.

Maybe it has to do with that he is Spymaster, Heir to another nation (Egypt) or Adopted himself ?
I had same issue at some point, but at the age of 60 I managed to get a bastard with a young lover. Legalized him later to be my heir.
Then he appeared to be 'Gay' which showed a -50% modifier Chance/Year to have children. So now it became even more unlikely to get a new heir ;p
A secret lover can happen through an event. Will be a bastard then.
Otherwise you wiill need to divorce first, before you can select any of the 'marriage offers' again. Those indeed will always give you 2 options to choose from. New chance at least or stick to the old one and hope for a shot of luck.
