I read that the zodiac spear is no longer missable?
Is that true in both the regular play through AND the Trial mode? Can you always just get a zodiac spear?

Is the same true for the rest of the permanent missables?

Correct, Zodiac Spear is now always in a 1% respawning chest in Henne Mines, so you can get as many as you like. It's also available through the Hunt Club (though that one is missable).

There's very few items now that can actually be missed. Random quest/completionist stuff aside, it's mostly just Genji Gloves (two available, both missable: steal from Gilgamesh and Hunt Club reward).
